The instructions on this page will help you to understand the AP process and assist you to complete either the My Progress and Support form (level 1 & 2) or My Academic Progress Response form (level 3).

Please note, it is important you submit your form by the due date. Level 3 students, if you do not submit your form by the due date, you are at risk of being excluded with no right of appeal.


My Progress and Support - Level 1 & 2:

  1. Look at the Academic Progress PowerPoint presentation
  2. Read the Notice of Unsatisfactory Progress page
  3. Fill in your My Progress and Support and submit your form by the due date

My Plan for Success - Level 3:

  1. Look at the Academic Progress PowerPoint presentation
  2. Read the Notice of Unsatisfactory Progress page
  3. Read the FAQ flyer
  4. Fill in your My Academic Progress Response and save it as a draft
  5. Gather your supporting documents (if you are not able to get all the documents, you can submit them to the faculty at a later date)
  6. Email your draft My Academic Progress Response and documents to studentrights@monsu.org for confidential and independent feedback
  7. Make any edits and submit your form by the due date
